Please forward the following information: 1) Artist, Organization or Business contact information: name, address, phone number, fax number, e-mail address, and (include your web site address, if relevant); 2) Professional or Amateur: are you a professional artist or arts organization (create art as your livelihood) or are you an amateur artist (create art as a hobby); 3) Type of artist or art produced: Visual Artist--e.g. Sculpture, Oil Paint, and etc., Performing Artist--e.g. Music, Theatre, Dance, Multi-media and etc., Crafts Person--e.g. Stained Glass, Wood Work, Jewelry, and etc; 4) Promotion--list methods used to publicize your work (web-site, press releases, mailings, King County or other artist roster, word of mouth, and/or other (please specify).
Send the above information to recreation@desmoineswa.gov or fill out the Des Moines Cultural Register Form available at www.desmoineswa.gov in the Forms Section (scroll down to the Parks and Recreation Forms Library). Send the completed form to Des Moines Parks, Recreation and Senior Services Department 1000 S. 220th Street Des Moines, WA 98198.
